the medicines were not bringing any positive improvement in Johnsy because she refused to eat any food and had lost all will to live. She held to the firm belief that she would not recover from her pneumonia and would die when the last leaf of the ivy vine fell.

The doctor believed that her chances of recovery were low as she did not show any willingness or determination to live.
